Annual Physical Examination Preparation Checklist
Before the Examination
1. Understand the Purpose: Know the reason for your annual physical examination and the value it brings to your overall health.
2. Research: Understand the standard procedures and tests involved in a physical examination.
3. Insurance: Verify that your insurance covers your annual physical examination.

7. Medical History: Make a note of your complete medical history, including any surgeries, hospitalizations, allergies, and chronic conditions.
8. Family Medical History: Write down any serious illnesses that your close family members have had and their age when they were diagnosed. This includes parents, siblings, and grandparents.
9. Health Changes: Write down any recent changes in your health. This includes symptoms like fatigue, pain, dizziness, changes in weight, changes in sleep patterns, etc.

On the Day of the Examination
11. Arrive Early: Arrive 10-15 minutes early to complete any necessary paperwork.
12. Identification and Insurance Card: Bring your ID and insurance card to the clinic.
13. Comfortable Clothing: Wear comfortable clothing that is easy to take off and put on.
14. Bring Your List of Medications: Remember to bring your prepared list of current medications.

After the Examination
